a 72 year old man comes to A&E with history of falling to the left arm. He has pain and unable to move his UL. he also told you that he had difficulty in passing urine for one week.
questions were :
- how do you proceed from that
- physical examination?

27 year old man. with loss of shoulder contour, muscle wasting over deltoid region
shoulder adduction with extension and pronation of elbow and flexion of fingers at PIP joint.
questions were :
- physical examination
- show what is grade 2 and grade 3 muscle power of the elbow
- diagnosis ? complete brachial plexus injury
- differences between pre and post ganglionic? why it matters?
- differences between complete BPI and partial BPI?
- how do you manage this patient?
- what kind of rehab do you want to do and why?
conclusion : *sigh*
thursday, written paper
a 72 year old lady came to A&E with history of falling with an outstretched right hand. she complained of pain and swelling over her right hand. she had dinner fork deformity. X-ray showed Colles' fracture of right hand with obvious osteoporosis. she was treated conservatively with closed manipulation and reduction (CMR) under sedation.
questions were :
- what is Colles' fracture
- how do you advice the patient before doing CMR
- what sedation do you give? how you administer it and how you monitor the patient during CMR
- explain how you perform CMR for Colles fracture and what type of cast
- what do you do after CMR
- how do you advice the patient before you discharge the patient
- what are you plan for follow up
- briefly explain general non pharmacological and pharmacological management of osteoporosis
conclusion : berganda-ganda *sigh*. keluar awal sbb tatau nak jawab pe ntah.
